Summary:
Provides administrative support to RN and Director.
Will provide direct care to residents following the individual service plan.
Serves as a mentor and resource person to other care staff.
Will be on on-call rotation schedule and assists in providing staffing schedule and coverage.
Essential Duties and
Responsibilities:
1.
Assists residents with activities of daily living, including bathing, dressing, grooming, toileting, transferring and getting to and from activities and meals according to the individual service plan.
Allows and encourages residents be as independent as possible while following the resident's care plan.
2.
Serves meals to residents in the dining room or apartments.
May assist in preparing meals following pre-planned menus, encourage residents and tenants to eat a proper diet and take adequate fluids while respecting the resident right to free choice.
3.
Initiates and participates in leisure activities provided for residents as described in the activity calendar.
Encourages residents to socialize and participate in planned activities or programs and to develop friendships with other residents.
4.
Documents in resident records on each shift and in accordance with company policy and procedures.
This includes documentation related to:
medication distribution; leisure activities; incidents/events and observations; charting any changes in physical condition and/or behavior; visitors; changes in eating, ambulation, toileting etc.
Observes and documents the health and emotional condition of each resident, promptly reporting all changes to the supervisor and/or on-call.
5.
Must be able to pass medications accurately and as ordered by physician.
Assist residents with medication as defined in medication procedure.
On each shift documents on each resident who self administers medication.
Performs medication audit of narcotics and other mood altering drugs on each shift.
6.
Maintains a clean, safe and orderly environment.
May performs general housekeeping following cleaning schedules.
Cleaning of bedrooms, apartments, dining area, living space, bathrooms, kitchen, and public areas, etc.
7.
Changes bed linens, washes resident laundry per individual service plan.
8.
Responds promptly and positively to resident requests for assistance, including emergency systems, telephone calls, and requests from family and friends.
Always giving great customer service as a response.
9.
Follows proper procedures in emergency situations (fire, severe weather, missing person, etc), obtaining additional assistance if necessary.
Must have the emotional and physical ability to handle emergencies.

Weekly reviews medication cart for PRN medications, such as eye or ear drops, lotions, saves, etc so a supply is on hand and does not run out.

Weekly does a check on resident's medication to ensure that:
Staff are punching/administering medications properly Staff are destroying medications properly Current med orders are on file.
Discontinued medication orders are removed from staff medication binder.
Discontinued medications are removed from cart Labels are correct on each medications that has been prescribed By month end all resident medication program should be reviewed.
12.
Perform daily audit of controlled medications and sign off.
13.
Creates and maintains the monthly staff schedule.
Posts the staff schedule no later than two weeks prior to the next schedule starting.
Master copy of staff schedule is maintained in a separate file so at any given time there is an original schedule showing which staff worked when.
14.
Performs weekly walk through of interior and exterior of building to ensure all safety and maintenance type issues are documented and addressed promptly.
15.
Follows up weekly on resident concerns to ensure they have been documented and taken care of following RN and Administrator instruction.
16.
Assists with staff training/orientation and shadowing of new Resident Care Assistants.
17.
Does not get involved with gossip, promotes positive can do attitude with management and co-workers.
18.
Assist RN and Director as needed.
19.
Lead must be at work, on time, dressed appropriately, and follow company policies and procedures as they are the mentor to other staff.
20.
Updates and maintains the State requirement documents for licensing visits.
21.
Processes accounts payable on a weekly basis for Director signature and mails.
Ensures Accounts Payable is timely and accurate.
22.
RN and Director trains lead in meeting deadlines of filling out weekly paperwork whenRN and Director is absent from work.
23.
May be required to use personal vehicle for business purposes.

Qualifications:

High school diploma or general education degree (GED) preferred; and 2 years related experience and/or training; or equivalent combination of education and experience.
Experience working with frail elderly and individuals with dementia in a CBRF.
Ability to read and comprehend simple instructions, short correspondence and memos.
Ability to write simple correspondence.
Ability to effectively present information in one-on-one and small group situations to residents, residents' families and other employees of the company.
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Ability to compute rate, ratio, and percentages.
Ability to apply common sense understanding to carry out detailed but uninvolved written or oral instructions.
Ability to deal with problems involving a few concrete variables in standardized situations.
Uses good judgement in emergency situations and get help when needed.
